Can you help me ???

I'm doing my first project . It is about Medical Mistakes, so I want to do a questionnaire, but I Don't know the question that required in the questionnaire

I already have did the introduction of questionnaire .

The introduction is :

Medical mistakes are a damage caused because of doctors failure in doing his dues or diagnoses of the illness

I want from you help me to give me the questions .thank you for help me .

good luck and i found a website which I hope it could help you.. it's about your subject and it has a lot of information and questions but unfortunately it's in arabic ,however it'll be helpful....

?what are the main causes of medicals mistakes

do u think the bad equipments which are using in hospital is the cause
? of medical mistake
